This is restricted to central and southern Appalachian regions, usually on damp acid soils with Rhododendron maximum. The coll. attributed to HARL (MM for WKY) was made close to, and possibly within, the BELL county line. If cpmbined with Neottia (K, W), the name would be N. smallii (Wiegand) Szlachetko.
